Please Join us at Kindred Transitional Care & Rehabilitation located at 581 Poquonock Ave in Windsor for a Presentation on “What to expect with Home Care Services” on Tuesday, September 15 from 5:00pm - 7:00pm.

The presentation, provided by New England Home Care will talk about how to be prepared for the transition home from a short term rehabilitation Facility or Hospital to avoid Re-hospitalization. Also understanding the scope of Services Available to make your transition successful: Nursing Services, PT, OT, and Home Health Aides. Refreshments will be served.
